Dermal Fillers

Dermal Fillers

As we get older we lose facial volume; our bony underlying skeleton recedes and the overlying fat pads shrink. This loss of structural support for the overlying skin envelope results in deflation, descent and skin wrinkles.

What are Dermal Fillers?

Hyaluronic (HA) fillers help restore structural support by imparting volume to the skin envelope.  At the Van Vliet Clinic in Newmarket, dermal fillers such as Juvederm®, Restylane®, Sculptra®, and Teosyal® are a used to add volume and structural support to aging skin, restoring contour and smoothness. For facial dermal fillers, Dr. vanVliet is an expert injector.  She cherry picks and uses only the latest and best dermal fillers.  These safe and effective fillers are FDA-approved for facial rejuvenation. Although they have differing properties, Dr. vanVliet carefully chooses which ones will serve you best. Her experience and soft touch will have you looking immediately like your best rested and replenished self.

As we get older we lose facial volume; our bony underlying skeleton recedes and the overlying fat pads shrink. This loss of structural support for the overlying skin envelope results in deflation, descent and skin wrinkles. Fillers help restore structural support by imparting volume to the skin envelope. These fillers have differing viscosities and properties but essentially they serve to lift and re-contour the face to smooth out wrinkles. Dr. vanVliet uses a gentle hand and discernment to give you a seamless natural-looking result.

When it comes to fillers, people often have a real fear of looking weird or aberrant –  and justifiably so. We have all seen duck lips and inner tube lips that detract significantly from a person’s appearance. The poor results we see usually have less to do with the fillers used than the injector’s skill – or lack thereof.
While most dermal fillers available in Canada are themselves very safe, it is extremely important to choose your injector well. Dr. vanVliet has extensive training and experience with facial anatomy and injection techniques. This, combined with her strong aesthetic sensibilities and a gentle hand, will give you a seamless, subtle, and very natural-looking result. Her goal is to have you looking naturally your best.

Young patients seeking dermal fillers are usually looking to augment or enhance a facial feature such as their cheekbones, lips or chin. They still have lots of facial volume and their skin is smooth and taut, so fillers serve a different function in this age group than they would for patients in their middle years.

The most common areas for dermal fillers in patients 35 years and up are:

  • Fine lines and wrinkles around the lips, nose, and eyes: Most superficial wrinkles around the face can benefit from fillers. Dr. vanVliet will commonly use smooth, fine fillers such as Restylane Fine Lines® or Teosyal Redensity II® for taking care of fine skin wrinkles.
  • Tear trough and lower eyelids: Sunken tear troughs or under-eye bags can be diminished by adding volume to depressed areas and evening out the skin’s contours. This can be a challenging area to correct for.  Dr. vanVliet uses a fine micro-cannula and Teosyal Redensity II® filler here to reduce the potential for swelling or a ‘tyndal effect’.
  • Cheek volume: Restoring contour and volume to depleted cheeks is Dr. vanVliet’s most common filler treatment. It instantly brings volume back to the upper face, like when we were young, framing the eyes beautifully. It can turn back the clock instantly! Dr. vanVliet’s favourite filler here is Voluma® from the  Juvederm® family. This filler has excellent lifting and volumizing properties that can lift and restore volume to the mid face with results that can last from 18-24 months.

While they are both injectable, and they are both offered as a means of fixing wrinkles, dermal fillers replace lost volume underneath the skin to smooth out wrinkles from below the skin’s surface, effectively “sculpting and filling” wrinkles from the inside out.

Wrinkle relaxers, such as Botox® and Dysport®, target the wrinkles that are caused by the repetitive movement of facial muscles, effectively suspending their movement while allowing the overlaid skin to relax and smooth out.

When a board-certified plastic surgeon or dermatologist is your injector, you are paying for their clinical acumen as well as their expertise — not just your syringe of filler. Dr. vanVliet is an expert injector and she does all of her own injections, rather than using a second party. Her approach is gentle and nuanced. She understands that everyone’s face is unique; there are no cookie-cutter solutions. Therefore, pricing for wrinkle relaxers in her practice is generally based on her time, as well as how much product has been injected. Treatment plans and cost are determined upon your consultation with Dr. vanVliet.

What is hyaluronic acid?
Hyaluronic acid (HA) fillers are jelly-like substances that can be injected at various levels into the skin, depending on the results we are trying to achieve. HA is actually a naturally occurring substance found in our bodies (eyes, joints, and skin), but it tends to get depleted as we age. It absorbs water like a sponge, bringing necessary moisture and volume to the skin. Babies, for example, have plenty of HA in their skin, giving them that dewy, smooth skin you just want to hug.
HA-based fillers are easily accepted by the body with very low complication rates. They are slowly metabolized and naturally absorbed by your body over time. Dermal fillers can last anywhere from 6 to 18 months depending on the type of filler used and where it has been placed.
Are fillers safe?
Safety is an obvious concern for patients undertaking injectable treatments with fillers. Dr. vanVliet uses only FDA-approved, safe, hyaluronic acid-based fillers that are absorbable. The body easily tolerates fillers, and complication rates are very low.
They are also easily reversible with hyaluronidase injections that instantly absorb the fillers should the need arise. While Dr. vanVliet has never used this product on any of her own patients, she has used hyaluronidase on unhappy patients who have come to her from elsewhere, needing correction from poor results.
Are there any risks?
In the right hands, dermal fillers are very safe non-surgical treatments. Dr. vanVliet is a very skilled technician, and she is conservative with her injections. Her recommendation is to err on the side of under-correcting rather than over-correcting a problem area. This will significantly reduce your risk of an unnatural outcome.
Bruising occasionally occurs, and the risks can be reduced by staying off all blood-thinners such as Aspirin® a week prior to your treatment and taking it easy the day following your treatment. Depending on the extent of the work, you can anticipate some mild swelling, but usually this settles within 7 to 10 days. Dr. vanVliet uses sterile techniques, so infection rates are very low. The risk of injection into or around a blood vessel causing tissue damage or potential loss of vision is rare.
Will the injections hurt?
Fear of pain with facial injections is real. Dr. vanVliet uses ultra-fine needles to reduce your discomfort as well as your risk for bruising. She will often use topical freezing for sensitive areas like the lips. As well, all of the hyaluronic based fillers that she uses come already combined with freezing to reduce injection pain. Most patients find this an easily tolerable procedure.
Are there any restrictions following filler injections?
The recovery time following filler injections is minimal, and the only restrictions on the day of your injections are:

  • No exercise or overheating yourself after your injection session
  • No massage or pressure on injection sites or head- down maneuvers for 4 hours

After your treatment session, you can resume all your normal activities. Many people return to their day job or other activities.

Dermal fillers are not the only wrinkle correctors available. You could also be a candidate for other non-surgical anti-aging treatment options including: light and laser treatments, Botox® and Dysport® wrinkle relaxers, and topical cosmeceuticals. Dr. vanVliet is very comprehensive and holistic in her assessments and in her skin rejuvenation treatments; she keeps current on the most effective, cutting-edge, non-surgical rejuvenation treatment options.
